AGING IN PLACE is a phrase used to describe a person living in the residence of their choice, for as long as they are able, as they age. This includes having hospitality services, amenities, and health care options available as needs change over time. Since 2018, this collaboration has facilitated the development of educational programming, resource partnerships, and service deliverables that take into consideration the existing health care priorities of residents today and those anticipated in the future. Outcomes have been far-reaching and include:

  • “Driving your Well-being” education on topics related to “Life’s Intersections” such as Being Mortal, Loss of Dreams & Rebuilding of Dreams, Greif & Hope, Hospice & Palliative Care.
  • The Introduction of the GrandPad Senior communication technology to improve resident social outreach and engagement by simplifying smartphone-like communications and application use.
  • A working relationship with a prominent MN-based health care system that provides social working expertise and resource direction over the phone, in-person, and online to help caregivers along their journey caring for a loved one.
  • A brochure rack of in-house hospitality, home health care, medical supply, housing identification, and moving company services to support ever-changing needs.
  • Education on in-home INR lab testing so residents can eliminate the need for repetitive outbound clinic visits.
  • Partnerships with neighboring senior living communities to share occupancy data, best practices, and care delivery choices for residents and families who may require transitional care or advanced assisted living and memory care services.

The challenge families often face is a commitment to a 3 to 4-hour care package minimum offered by a care provider. Often residents do not need as much care but are still required to pay for it. The best game-changer from RosePointe’s Optimizing Aging in Place initiative in 2019 was a pilot partnership with Comfort Keepers, one of Minnesota’s top 150 companies to work for.

AFFORDABLE HOME HEALTH CARE – A FIRST OF ITS KIND   RosePointe has helped many families with recommendations for in-house home health care when needs arise. The support spectrum offered by these agencies ranges from companion care to skilled nursing to hospice. Private pay contracts are negotiated by families with an agency of choice and exceptional services have been delivered by companies such as Comfort Keepers, Visiting Angels, Home Instead, and other hospital-aligned agencies.

Residents now have an affordable shared cost model where home health care services can be delivered across multiple residents 7 days per week, four hours in the a.m., and four hours in the p.m. with contracts for a minimum of 60 minutes for $40.
